Vaginal surgery for vaginal enlargement and looseness and no more sexual orgasm

My problem is that my partner complains of my vagina being loose and it is very hard for him to have an orgasm unless i am on top, it doesn't seem to be possible any other way due to this vainal laxity. I have had four children, the first two being over 8lbs and 17mths apart (if that has anything to do with it).. I had a vaginal repair two years ago of which things were better for about the first six months, now its back to how it was. My gyno said there is another surgery i could have using a mesh and having another vaginal repair however i don't think he really understands either as he said can't i just change my sexual positions of which i have and the only time my partner gets any satisfaction is with me on top.
