I'm 30 years old, unmarried, but I feel myself like an aged old person. In next 6 months I'm going to get married but worrying about my health.

To be specific I'm suffering from following symptoms:

1. Daily lower back, neck/shoulder pain.
2. Occasionally penis, testicals, rear brain or whole head pain.
3. Depression, stress , anxiety, and emotional instability.
4. Eye floaters since age 16-17.
5. Weak eyesight (I use glasses 2.25 near-sighted).
6. Occasionally sun-light sensitive eyes.
7. Attention Deficiency and Absence mind. Most of the time I cannot concentrate on some issue being discussed in some group discussion. Also I lose memory very fast. It was not the case before 6-7 years ago. But now I forget about things very frequently.

11. Exhaustion and fatigue. I feel too much exhausted after 12-48 hours of ejectulation.
12. Unwanted penile bending. My penis is curved downward about 20-25 degrees after one inch from root. Also root of penis (1 inch before start of curve) is not as thick as rest of penis. I want to make my penis straight and thick and long in length.
13. Premature ejaculation. I'm too much worried about this problem. I can masterbate only for half to one before ejeculation.
14. I've symptoms of premature hair loss or decoloring (I've lot of gray hair). Naturally I've black hair.

21. I don't know if over-masterbation is the reason or not, I'm feel pain in my anus (may be it is hemorrhoid). Sometimes pain gets swear. I'm feel ashmed to go doctor and tell about this. I'll appreciate if you suggest some medicine for this problem too.
