Reader: 12/01/2003> Dear Doctor, I am 30 and have been addicted to internet pornography and masterbation for the past 3 years, which I have used as an outlet for stress. I have a doctorate in organic chemistry and an interest in brain chemistry. I have not had a steady girlfriend in this time but have from time to time had a girlfriend and had sex for a month here and a month there. For 2 years of the 3 years I always smoked marijuana when I masterbated, which I did 2-4 times a day. I then gave up marijuana and have not smoked any for a year. I stopped mainly because I was not getting morning erections anymore and I had trouble keeping good erections during sex. I was also having trouble sleeping, waking up at 5am in the morning with a racing mind and not being able to sleep. The following 9 months I masterbated about twice a day, smoking cigerettes when I masterbated, slowly cutting down to masterbating about once a day. I now do not smoke any cigarettes and masterbate probally about 4-6 times a week. Sometimes I can manage a few days not masterbating (I have never managed more than a week) but then I tend to sort of binge. Currently I find that I am rather anxious and stressed. I am still having trouble sleeping, waking up at 5am and rarely being able to sleep any further. Refraining from masterbating is very difficult for me. I rarely get morning erections. I can usually get erections on my own but they sometimes are not particually rigid. When I have sex, the erections are not very rigid and I tend to prematurely ejaculate. This is very upsetting for me as up until 15 months ago I had great erections, rarely had premature ejaculation and could have sex 3-4 times in an evening. I have now lost 2 girlfriends because of this problem. I am currently seeing a girl but I am starting to put off having sex or at least try to work it so that I have not masterbated for a few days beforehand. Also I can only manage it once in an evening. I have not told my GP about this addiction but he has offered me SSRIs for my anxiety which I am reluctant to take. I am thinking of taking them now as the serotonin boost might help my premature ejaculation or drug induced impotence might stop me masterbating. Perhaps it's even an OCD! I did tell him about the impotence and he thought it was down to anxiety. He gave me some Viagra to try and give me confidence. It worked quite well the first time but not so good the second time. I have tried 5-HTP(50mg a day, for about two weeks) but this does not seem to really help and makes me a bit anxious when I take it. Incidently, how long would you have to take 5-HTP for it to have a noticeable impact akin to anti-depressants.
